Dr. Sung Kim, PT, DPT, TPI-Certified
Dr. Sung Kim graduated from California State University, Northridge in 2003 with his Bachelor of Science in Kinesiology with an emphasis in Athletic Training. He continued to pursue his education and develop his training by earning his Doctorate of Physical Therapy from the University of Southern California in May 2007. As a former athlete, he has extensive experience and understanding of sports-related injuries and what it takes to return his patients back to doing what they love. High school and collegiate athletes, weekend warriors, ballet dancers, military personnel, and even professional athletes have benefited from Dr. Kim’s treatment approach to restore their functional capabilities. Dr. Kim specializes in treating patients with hip and knee pain, as well as most common shoulder injuries. Dr. Kim strongly believes in a holistic approach to treating the person, not just the injury or diagnosis. He utilizes every avenue of treatment available, including manual therapy, joint mobilization techniques, therapeutic exercises, modalities, kinesiotape, and most importantly, patient education. As a Southern California native, Dr. Kim enjoys numerous outdoor activities such as hiking, swimming, playing volleyball, basketball, snowboarding, and mountain biking.
Dr. Thomas Lin graduated from the University of California, San Diego with his Bachelor of Arts in Human Development in 2001. His participation on the university track and field team, and interest in injury rehabilitation led him to pursue an education in physical therapy. He went on to graduate from the University of Southern California with his Doctorate of Physical Therapy in May 2006. Dr. Lin has substantial training and continuing education with HVLA (High Velocity, Low Amplitude) techniques for treatment of spine-related disorders and injuries. He employs a wide variety of manual therapy techniques and corrective exercise programs to treat neck and back pain. Dr. Lin also specializes in treating patients with TMJ dysfunction and headaches, having received continuing education from the University of Southern California. For over 10 years, Dr. Lin has treated patients in the San Fernando Valley and in the process, has developed a strong relationship with many healthcare professionals and physicians in the area. During his free time, Dr. Lin enjoys running, playing basketball, and exercising with his wife. He also likes to spend time taking his children to the park and story time at the library.

Dr. Leroy Velasco, PT, DPT
Dr. Leroy Velasco graduated from the University of California, Riverside with his Bachelor of Science in Biological Sciences in 2011. He then spent several years broadening his experience and exposure to physical therapy and rehabilitation by working as the Lead Exercise Specialist at a local physical therapy clinic. Dr. Velasco then decided that he wanted to pursue his passion and education by earning his Doctorate of Physical Therapy from the University of St. Augustine in San Diego in 2017. His clinical interests and specialty included working with various orthopedic conditions, sports-related injuries and pathological conditions of the foot and ankle. He plans on furthering his skills and technique by earning his Orthopedic Clinical Specialist (OCS) certification and becoming a member of the Institute of Physical Arts. Dr. Velasco is very excited to join the team at BASE Training and Physical Therapy and is looking forward to working with the surrounding community. In his free time, Dr. Velasco enjoys surfing, biking, playing tennis, soccer, and golf, and participate in rock climbing.
Dr. Samantha Ruby, PT, DPT
Dr. Samantha Ruby graduated from Lycoming College in Pennsylvania in 2010 with her Bachelor of Arts in Psychology with a Minor in Biology. She discovered her passion for injury rehabilitation through her athletic background in basketball at the high school and collegiate levels, as she utilized her personal drive and compassion to return to the court following numerous knee injuries. After gaining experience as a physical therapy aide, she went on to earn her Doctorate of Physical Therapy from Thomas Jefferson University in 2013. Since graduating, she has expanded her skill-set through years of experience in a Level 1 trauma center, where she worked with neurological, orthopedic, post-surgical, cardiac, and generalized weakness diagnoses, amongst others. She received advanced training in LSVT- BIG to better serve individuals diagnosed with Parkinson’s Disease and was part of a pilot team that introduced physical therapy services into the Emergency Department setting. Her clinical interests include combining her wide variety of skills from each setting to provide every patient she meets with an individualized and holistic approach that specifically targets his or her goals. She is also a 200+ hour certified yoga instructor and enjoys combining her yoga background with her education in rehabilitation. In her free time, Dr. Ruby enjoys hiking, doing yoga, running, biking, and taking advantage of all the sunshine that her recent move to California provides. She is very excited to join the team at BASE Training and Physical Therapy and is looking forward to sharing her expertise with the surrounding community.
Dr. Adrian Chavez, PT, DPT
Dr. Adrian Chavez graduated from California Lutheran University in 2016 with a Bachelor of Science degree in Exercise Science with an emphasis in Health Professions. From there he went on to attend the University of Southern California where he received his Doctorate in Physical Therapy in 2019. In his time at USC, Adrian was able to work in clinical rotations ranging from the Acute Rehab Unit at Keck Medical Center to the USC faculty clinic on Marengo. Adrian has had the opportunity to work with professional baseball players; performing physicals for the Los Angeles Angels at Spring Training in Tempe, Arizona. He enjoys working alongside all athletes, ranging from young to old. Adrian enjoys combining manual therapy techniques and therapeutic exercise from all schools of thought to provide an individualized and tailored program for all his patients. In his free time, he enjoys running, mixed martial arts, and ultimate frisbee.
Jordan Jonker, MOT, OTR/L
Jordan received her Bachelor of Arts in Sociology from the University of Minnesota and her Masters of Occupational Therapy from Texas Woman’s University. Jordan’s journey into the field of occupational therapy began in middle school with an injury to her hand that required extensive hand therapy to heal properly. It was that incident, paired with her fascination for the human body and passion for helping others that drew her to the profession. Jordan is a licensed Occupational Therapist who specializes in upper extremity conditions and postural rehabilitation. She currently holds her license to practice in both California and Minnesota and holds certifications in Basic Life Support and Mental Health First Aid. As an occupational therapist, Jordan works with patients of all strengths and ability levels to help them be as independent and successful in their day-to-day lives as possible. She uses her unique skill set to design patient-centered and holistic treatment plans to effectively address various conditions throughout the body. Before joining BASE, she spent time working with patients in various settings including education, outpatient pediatrics, outpatient orthopedics, hand therapy, inpatient rehab, acute care, critical care, behavioral health, and home health. Jordan was born and raised in Minnesota, but has since lived in Texas, Utah, and California. Outside of the clinic, Jordan enjoys running, hiking, doing yoga, and spending time with her fiancé and their two dogs, Finley and Howie.
Eddie Park graduated from California State University, Northridge with a Bachelor of Science in Kinesiology with an emphasis on Exercise Science. From a young age, Eddie’s life has been centered around physical activity and sports. Whether it was playing organized football in high school or just recreational golfing or snowboarding with friends on the weekends, he was always staying busy and active. Throughout his college career, Eddie developed a natural curiosity for human anatomy and physiology, as well as how the body responds to exercise and physical exertion. Since obtaining his Personal Training Certificate, Eddie has also committed to pursuing his studies to become a Certified Strength and Conditioning Specialist (CSCS) to help people of all ages and activity levels reach their full potential and achieve their personal goals. His passion to help others drives him to learn and continuously grow in the health and fitness arena.

Jessell graduated from California State University, Northridge in 2013 with a Bachelor of Science in Kinesiology. Jessell joined BASE Training and Physical Therapy several years ago bringing with her a strong background in management, customer service, and healthcare admitting. With this wealth of knowledge, since being with us, has become the roots of the practice. She is the person behind the scenes, but often assists with all aspects of patient care. Jessell also handles recruiting and hiring new employees, marketing, and insurance billing in our office. Outside of the office, you will find Jessell decorating her home, traveling, and walking her two dogs, Louie and Maddox.

Lindsay received her Bachelor of Arts Degree in Psychology at University of California, Riverside. During her college career, she volunteered with Inland Empire Lighthouse of the Blind where she assisted various programs to aid patients' daily activities, as well as organized and hosted social events. Lindsay aspires to become an Occupational Therapist. In her free time she enjoys baking, trying new restaurants, and enjoys spending time with friends and family.

Megan received her Bachelor of Science Degree in Health Administration in May 2020 from California State University, Northridge. With a strong background in customer service, Megan also hopes to pursue her Masters in Health Administration in the coming years. While caring for her grandmother after she fell ill, Megan was inspired to start a career in health care to ensure the general population has access to high quality care. In her free time, she enjoys cooking, going to the beach, and spending time with her son and family.
Hana graduated from California State University, Northridge in 2019 with a Bachelor's in Kinesiology with an emphasis in Sport Studies. She fell in love with sports at a very young age due to her years of involvement in softball and cheer. Reasons why she wants to pursue a career within Physical Therapy is because of her own personal experience receiving treatment due to a sport related injury and seeing first-hand how physical rehabilitation improved her grandma’s quality of life. During her free time, Hana loves going to the beach, snowboarding, and wakeboarding.
Yasmine is currently pursuing her Bachelor of Science in Molecular Cell and Developmental Biology Degree at University of Southern California, Santa Cruz. She recently held a position as a Research Intern and participated in multiple research studies with Project Baseline. Yasmine aspires to pursue a career as a Physician Assistant. In her free time, Yasmine watches movies on Netflix and spends time with her family, friends, and dogs, Sepia and Prince.

Jasmine received her Associates Degree in General Science and Liberal Arts at Santa Monica College. She previously worked as a Companion Care Volunteer for Geriatrics Unit at The UCLA Medical Center for several years. Jasmine is a certified Emergency Medical Technician and is currently pursuing her career as a Gerontology Nurse. In her free time she enjoys hiking, yoga, and spending time with her corgi, Pepper.

Kevin is currently attending California State University, Northridge to pursue his Bachelor's Degree in Kinesiology. He is entering his 11th year competing in Track and Field at the division 1/ post-collegiate level and currently holds a USATF Level 1 Coaching Certification. After gaining knowledge about managing injuries throughout his athletic career, he aspires to become a physical therapist to help others get back to doing what they love and beyond.

Tyler graduated from California State University, Fullerton in 2016 with his Bachelor of Science in Kinesiology. Tyler has always been an avid sports enthusiast and enjoys playing a wide variety of sports. Throughout his educational career, he has competitively participated in basketball, volleyball, and baseball. Currently, Tyler is pursuing a career in physical therapy, and enjoys snowboarding in his free time.
