What type of insurance do you accept?

We are providers for Medicare, and most PPO insurances. Please call us if you have any questions regarding your insurance benefits.

 

What should I expect during my physical therapy treatment?

  • A thorough neuro-musculo-skeletal evaluation

  • Functional movement analysis

  • Manual therapy

  • Therapeutic modalities

  • Therapeutic exercises

  • Patient education and a home exercise program

 

What should I bring to my first visit?

  • Valid photo ID

  • Insurance cards

  • Physical therapy prescription/referral order from your physician

  • Medication list and any operative reports

  • Reports from any diagnostic tests (For example: X-rays, MRI, EMG)

  • Comfortable and loose fitting clothing and exercise shoes

  • Any braces, prosthesis, or assistive devices you are using

  • New Patient Registration forms - for your convenience, available to download and print from home

 

Where do I park?

BASE Training and Physical Therapy offers plenty of free parking behind the Plaza De Oro shopping center. We are located upstairs on the second floor. For your convenience, there is an elevator in the middle of the Plaza, as well as several staircases leading to the second floor. 

 

How long is each appointment?

Your initial evaluation and subsequent treatments will last approximately 60 minutes. 

 

What is a D.P.T.?

D.P.T. stands for Doctor of Physical Therapy, and is the highest degree that one can achieve in the physical therapy profession. All physical therapists at BASE Training and Physical Therapy have received their Doctorate of Physical Therapy. We are the industry leaders in movement analysis and non-invasive pain management.
